Someone in your city opens ChatGPT and asks where to get Botox. An answer comes back naming three practices. You are not one of them, and you have no idea why, because you rank on the first page of Google for that exact term.

Here is the short version. Ranking and being named are two different contests. These five reasons are what shows up most often when a practice that should be getting mentioned is not.

Reason One:

The Site Has No Structured Data

AI systems do not read a website the way a patient does. They look for machine-readable information that states plainly what the business is, where it operates, who provides treatment, and what is offered.

Most Med Spa sites have almost none of it. Maybe a basic business listing dropped in by an SEO plugin, which tells an AI system that a business exists at an address and nothing else. It does not connect the medical director to the practice. It does not connect the injector to the treatments she performs. It does not identify the service list as services.

Without that, an AI system looking for a Med Spa to recommend has to guess what the business is from paragraph text. It usually declines and names a competitor whose site did the work. Closing that gap is the core of AI search optimization.

Reason Two:

The Content Buries the Answer

Open your microneedling page and read the first two sentences. If they say something like “at our luxury Med Spa, we believe every client deserves to feel their best,” there is a problem.

AI systems pull short, direct answers. A page that opens with a real answer to the question that brought someone there is easy to quote. A page that opens with atmosphere gives them nothing to work with.

This is not a style preference. It is the difference between being quotable and being skipped. Say what the treatment is, who it is for, and what it does, inside the first hundred words. Be warm for the rest of the page.

Reason Three:

The Business Information Does Not Match

The website says one phone number. The Google Business Profile says another. An old directory listing still has the suite number from before the move. Facebook hours have not been touched since 2022.

Every one of those is a small conflict, and AI systems resolve conflicts by hedging. When information about a business does not agree with itself, the safest move is recommending a business whose details are consistent everywhere.

This is unglamorous work and it is frequently what unlocks everything else. Cleaning it up costs nothing but attention.

Reason Four:

The Reviews Went Quiet

180 reviews and a 4.8 sounds strong until you notice the most recent one is from last November.

AI systems weigh recency heavily, because a business collecting reviews every week is demonstrably active and a business that stopped might not be open. They also read the language inside reviews, not just the star count. A practice with 70 reviews where reviews are still arriving, mentioning specific treatments and providers by name, often gets named ahead of the practice with 180 stale ones.

Volume matters less than pulse.

Reason Five:

The Rankings Are Weaker Than They Look

This one stings a little.

AI systems draw heavily from pages that already perform in traditional search. A practice sitting fourth or fifth on page one, or ranking for the treatment name but not the treatment plus the city, is on the edge of what gets pulled into an answer.

Worse, many Med Spa sites have three or four pages competing for the same term because a blog post, a service page, and an old landing page all target “microneedling.” They split their own authority and none of them ranks properly. Fixing that usually produces faster movement than writing anything new, which is why traditional search rankings remain the foundation rather than the old way of doing things.

How to Check Where You Stand

Open ChatGPT and ask plainly for the best Med Spa in your city for the treatment that drives most of your revenue. Do the same in Perplexity. Then run a normal Google search and read the AI Overview at the top.

Write down who gets named. If it is the same two or three competitors every time, that is the answer, and it is fixable. None of the five problems above requires rebuilding a website. Most of them are cleanup.

The Part Nobody Says Out Loud

All of this works because it is true. Structured data that describes a practice accurately, content that answers real questions, consistent information, active reviews, and solid rankings are not tricks. They are what a well-run practice looks like when it is described honestly.

There is no version of this where a mediocre practice games its way into the answer and stays there. The businesses getting named are usually the ones doing good work with someone paying attention to how that work is described online.
